Headaches

Headaches are generally regarded as the symptom in the body with the greatest number of possible causes. Therefore to any doctor without specialist knowledge it is a nightmare! The excellent news is that the vast majority of headache sufferers we accept for treatment in our practice appear to respond very well. These headaches include “stress headaches”, “tension headaches”, cervicogenic headaches; occipitalgia; frontal headaches; and some types of migraines. Clinical trials around the world confirm that chiropractic treatment can be effective for these conditions.

However some do not respond... for example purely “hormonal migraines” with a definite monthly cycle, do not respond well and these cases are referred to other specialists who can hopefully help.

Interestingly Scott Middleton is married to his wife Helen because he cured a lady’s migraine headaches in 1977. The lady and her husband were friends of Scott’s then senior partner and wife at the time. Scott then met the lady concerned and her family at a Christmas party at his senior partner’s house. Many years later he married one of her daughters!

Please note that there is absolutely no truth whatsoever in the rumour that if his mother-in-law is unkind to him, Scott will ensure the migraines return!

Migraine

There are many different types of migraine... these include classical, common, cluster, abdominal, and even “migraine without a headache”.

Most migraines are multi-aetiological - in our experience they usually have about five factors that combine to cause the symptoms. Fortunately we find in-house that many migraines can be influenced and respond well to treatment and advice. Clinical trials have proven that chiropractic treatment can be a very effective method of treating migraine.

However “hormonal migraine” - ones linked to a definite monthly cycle - in our opinion, do not respond to chiropractic and therefore these patients cannot be accepted for treatment.

Occipitalgia

This is simply a headache felt over the back of the head. No complications and we find in-house that it regularly responds very well to precise, gentle treatment. Clinical trials around the world agree that chiropractic can be very effective for these headaches.

Tension "Stress" headaches

Stress does not cause the headache so this is really a misnomer... but it can aggravate the problem.

If stress really caused headaches then psychologists would have better results than chiropractors!!! Chiropractic can be very successful for the treatment of "stress-related" headaches" by curing the underlying problem. The ongoing "stress" that we all have then does not aggravate the pathology... stopping the headaches.

Neck problems

These can be chronic problems of many years duration, slowly becoming more and more restricted and painful... or agonising problems that suddenly start as incredibly severe pain in the neck (or base of the neck) with almost complete restriction of all neck movement. Clinical trials around the world have proven that chiropractic treatment can be effective for both acute and chronic neck pain. Many patients tell us that it came on for no reason”... but we usually work it out!

Disc prolapses

Every day we see many acute and chronic longstanding disc problems. Although there are usually 23 discs in the spine (it can vary!), the low back (lumbar) and neck (cervical) discs are the common ones to fail.

Discs can tear; bulge; protrude; or prolapse... with the lesion being of many shapes and sizes and located in a variety of directions. Disc problems can also occur in children as well as adults. The chiropractic profession is very well known for treating these very painful conditions... and we treat far more disc problems every year than any other profession. We are happy to report that the chiropractors at this practice are no exception!

NB. The first ever recorded diagnosis of a patient suffering from a “slipped disc” was a man named (Mr) G S Middleton... alas no relation of (Dr) S G Middleton... so Scott has absolutely no claims to fame on this one!

Torticollis

This presents as a very severe restriction of neck movement with agonising pain. The comments “I can’t even lift my head off the pillow” and “I can’t believe how painful it is!” are often mentioned!

It is common in children over the age of eight and at any age up to c. 60 year-olds - after which it becomes less common. We see about four cases of torticollis per week.

“Agony” is the correct word! We find chiropractic treatment works particularly well when combined with appropriate prescribed medication.

Whiplash

Alas a very common problem which appears to be on the increase as we have more and more enquiries about whiplash injuries each year.

See: Neck problems. When patients with neck problems and associated pains consult a chiropractor at our clinics they have the underlying problem fully explained to them and understand that with our gentle, precise treatment techniques we expect good results within a relatively short time.

Clinical evidence has shown that chiropractic treatment can give excellent results in acute and chronic neck pain so it is not surprising that the more up-to-date insurance companies, surgeons and solicitors are now referring many more patients with neck pains directly to us.

Obviously if there is an insurance claim then this would cover the cost of any necessary treatment and solicitors can authorise the payment without the patient outlaying any money themselves.

Dorsal or "thoracic" pain

Most dorsal pains are felt between the shoulder blades. Our last NHS audit found that we had 100% success for this region... (NB. 100% success is almost ludicrous and should never really be claimed... as it is extremely unlikely to ever be repeated!) So let’s be realistic... we normally expect good results treating pain in this region!

Frozen shoulder

One of the many common shoulder problems we encounter. In the classic mild to moderate cases the response to our treatment protocol is well proven in the clinical trials to be very good. Perhaps three or four treatments to improve mobility with the arm being able to fully abduct (to raise the arm out to the side above the head) followed by appropriate exercises to allow internal rotation (similar to doing a bra up at the back) to recover. Scott regularly liaises with orthopaedic surgeons and physiotherapists concerning this problem. Some frozen shoulders are very severe and thus require more invasive treatment from an orthopaedic surgeon. You will of course be advised if this is the case. See Shoulder pain

Rhomboid syndrome & Scapula pain

Often muscle related, but very often two or more concurrent conditions.

Scapula pain is felt in the muscles over the shoulder blade.

Normally it is a straightforward diagnosis and treatment.

It is often possible to teach the patient’s husband/wife/partner/parent/etc how to help by doing some treatment at home... so if possible please bring someone else with you.

Shoulder pain or weakness

Shoulders are also an area where diagnosis is paramount. There are literally dozens of different syndromes that cause pain in the shoulder. Because it is not a weight bearing joint, we see fewer cases of osteo-arthrosis in the shoulder than we do in the hip and knee. Shoulder pain with weakness or restriction is often misdiagnosed as “frozen shoulder”... Many pains are referred from elsewhere and we find that this region of the body is often misdiagnosed...

The combination of two or more types of treatment can often gain better results than just one approach... Always remember the maxim, if you are not responding after four treatments... do not continue with the treatment.

Radiating pain around the ribs to the side or front of the chest.

Usually straightforward to treat... NHS audited studies prove the effectiveness of our treatment.

Facet pains

Facet syndromes - typically felt as sharp, localised spinal pain.

A problem we see every hour in our clinics - without fail! This condition is not only seen more often than disc problems but it is the single most common problem we treat!

There are about four dozen facets that can get problems... in any combination... in any part of the spine... in all planes of movement... for a variety of reasons... each requiring careful, precise, individual treatment.

Low back pain... "lumbago"

The vast majority of our patients initially consult us because they are suffering from low back pain and related problems. These pains can be acute recent injuries or chronic longstanding problems. Many patients also have pains, pins and needles or numbness radiating into the groin or down the leg.

The causes of low back pain are many and varied... it can arise wholly or partly from congenital or developmental variations in structure; fracture or other local trauma; any one, or combination of a dozen facets joints; either sacro-iliac joint; discs; ligaments; blood vessels; abdominal/bowel problems; gynaecological problems; infections - eg TB; diseases eg. polymyalgia rheumatica or Crohn’s disease; degenerative disease; osteoporosis; any of the 170+ types of arthritis; central or lateral canal stenosis; primary or secondary tumours; secondary muscle spasm; leg length inequality; hip, knee or ankle/foot problems; etc etc, etc... ...the list continues... and is VERY long!

The good news is that 95% of low back pains are caused by complex mechanical problems

Consultant neurosurgeons and orthopaedic surgeons, chiropractors, pain specialists and rheumatologists are all able to diagnose the vast majority of low back problems... but our surgical and other medical colleagues are then unable to treat 90% of these problems except to ease the pains short-term, by giving strong drugs. This is because the causes of the pain are mechanical and are not severe enough to require surgery.

The British Government (CSAG) Guidelines in 1994 and the 2004 European Guidelines state that: "patients with low back pain should not have physical therapy such as ultrasound, interferential, traction, exercises, bed rest, hydrotherapy, etc..." "...or medication for more than four weeks."

These guidelines recommend expert manipulation as the best and most effective treatment.

The NICE (National Institute of Clinical Excellence) Guidelines agreed with the above comments and again recommended expert manipulation.

Patients should therefore be immediately referred for expert manipulation if they are not responding quickly (we recommend two weeks) to prescribed anti-inflammatory drugs.

Alas many GP’s are still not familiar with (and thus do not follow) the modern research protocols or the Government and NICE guidelines!

Academics in the scientific community know from the scientific and medical research that no other health professional has the qualifications, knowledge and skills of a chiropractor, nor their ability to clinically diagnose and expertly treat these problems using gentle, precise, highly skilled manipulative techniques.

Because diagnosis can be very complex it is important to mention every symptom to us. Some patients do not mention a symptom, thinking that it is not related... delaying the most appropriate treatment and therefore slowing down their recovery.

One, almost frightening, statistical survey found that of the patients who were not referred to us from an orthopaedic surgeon, neurosurgeon, pain specialist or rheumatologist... 95% did not have the correct diagnosis and therefore the advice and treatment they had been given prior to consulting us, was often not only wrong but also had delayed their recovery and, in some cases, made them much worse.

Please remember... In the majority of cases, if you do not have good improvement after four treatments, you should not continue having that type of treatment... because it is not working!!!

Sciatica

This is another extremely common condition which, along with low back pain, is the most common problem that patients are suffering from when they initially consult a chiropractor. Our practice is certainly no exception!

Typically this is pain radiating down the back or outside of the leg (or both legs), sometimes just down to the knee - but often right down to the foot (feet) and toes.

Neurogenic Stenosis

Stenosis of the Spine

Two different types of stenosis can be present. Often the patient cannot walk more than 10 metres without stopping. In extreme cases these patients will be too difficult for chiropractic care and thus require surgery. In less severe cases the pain can ease quickly and the walking distance improve dramatically. We will advise whether or not you need a surgical opinion."

Piriformis syndrome

A much “over diagnosed” condition! Since various other “practitioners” found out about this condition we regularly get disillusioned patients consulting us with this “diagnosis”... and still suffering pain after countless treatments!

But it can sometimes be a genuine diagnosis... in which case why wasn’t the patient much improved within a few treatments?

As always, it is important to consult someone who has experience in the treatment of this and similar conditions for an accurate diagnosis rather than wasting valuable time (and money!) on inappropriate treatment.

Spondylitis

The combination of an X-ray report showing spondylosis and no obvious cause for the pain... often results in a “diagnosis” of “spondylitis”. It is not really a diagnosis and the real underlying problem, when accurately diagnosed by a chiropractor, can often respond extremely well to treatment.

Spondylolisthesis

Usually found in the low back. Approximately 90% occur at the L5 vertebra - the lowest vertebra.

Usually found in the low back. The vertebra slips forwards on the one below. There are four grades found on X-ray - from the commonest “grade 1” to the more severe “grade 4”. It is often associated with spondylolysis, but in older patients it is often simply as a result of wear and tear.

It is often not a painful condition, but because it is present very close to where the pain is felt, it is mistakenly diagnosed as the cause of the pain. Chiropractors are very well equipped to correctly diagnose problems in this area and are expert in giving extremely precise treatment of the underlying problem - giving fast effective pain relief.

Spondylosis

An X-ray finding of wear and tear, a slightly reduced disc height and reduced curve in the neck or low back that is found in practically every single person over the age of 45 years. It has little or no relevance to pain. It is not a diagnosis and therefore it does not need treating! Clinical trials prove the effectiveness of chiropractic treatment for neck and low back pain.

Your chiropractor will examine you to diagnose the real cause of your pain and then advise you on the most appropriate treatment. Also see Spondylitis

Spondylolysis

A failure of bony fusion (-lysis) similar to a “fracture” in a vertebra (spondyl-) usually in the low back. It is rarely relevant to the pain. The real cause of the pain requires accurate diagnosis and advice on treatment... while the long-standing and largely irrelevant spondylolysis remains exactly the same... completely painless!

We see spondylolysis every day, because it is found in about 1 in every 20 patients!

Arm pain

Some pains in the arm can be from local injury or referred from elsewhere in the body.

This referred pain is usually the neck but can often be from more discrete injuries eg; the dorsal spine, heart, stomach,etc.

As always, diasgnosis is the key before specific treatment is given to eliminate the underlying problem.

Once the diagnosis has been made, the chiropractor will advise what treatment will be of most benefit.

Pins and needles or numbness

Pins and needles and numbness (paraesthesia) are usually caused by trapped nerves.

For over one hundred years, chiropractors have spent much of their working life "untrapping" these nerves and relieving the symptoms.

The chiropractic profession treats more trapped nerves every year than all the other medical professions put together.

This is done by using very specific, gentle adjustments of the relevent region of the body that was causing the problem.

The results are variable according to both the severity of the entrapment and which nerve is involved.

You will therefore be advised at the initial consultation of whether we can help and, if appropriate, the expected speed of recovery for your particular nerve injury.

As a general rule, nubmness of over two years duration will fail to respond to any treatment including surgery. Pain however can respond after decades of suffering!

Therefore if you have had numbness for quite a long time (and any treatment you have been having is not working) do not delay seeking expert advice or it may well be too late for us to help.

Sometimes pins and needles or numbness is caused by diseases. These would not respond to chiropractic care and these patients will be advised to consult their GP.

If appropriate, we will send the GP a written report advising about the underlying problem. The GP will of course then take over the management of the condition.

Tennis or Golfer's elbow

Very annoying pains... Notoriously difficult for anyone to predict the outcome of treatment and although clinical trials on tennis elbow prove that our treatment protocol can be effective... there are no trials on golfers elbow! Some respond spectacularly within a couple of treatments... some do not respond at all. This is a classic example of why a patient should not have more than a few treatments unless the initial results are good.

"Wear and tear" & "old age"

We have our limitations! However many patients find that with our gentle precise treatment they “rejuvenate” in the sense that they are less stiff; can put on their shoes and socks more easily; can rotate their necks to reverse the car better; and have less pain.

Please note that they still have the same amount of wear and tear “osteo-arthrosis” and will be exactly the same age... but they may well feel younger!

Abdominal pain

A surprisingly common complaint. If the GP or hospital consultant has not found any cause for the pain and the ultrasound scan/other tests are negative, then the pain is likely to be referred from elsewhere. Chiropractors will check the various causes of referred pain and then advise or treat accordingly.

Coccydynia

Pain in the coccyx

This is a very painful complaint. The last audit found that we see approximately twenty cases of coccydynia per week. The causes of coccydynia are many and varied and thus diagnosis is complex - as most have multiple causes.

Medical trials and Government guidelines recommend chiropractic treatment.

Groin pain

This is an incredibly common symptom. Many problems cause groin pain - including local injuries as well as referred pain from the spine, hips and other structures. Local problems such as hernias obviously need specialist surgical advice at the hospital. The majority of groin pains are fortunately referred from elsewhere. A recent survey found that every fifth patient consulting us (20%) did so because they were suffering from groin pain!

Treatment must be very specific to the underlying problem... hence, as always, accurate diagnosis is the key... followed by the correct treatment... resulting in fast effective relief of symptoms. After the diagnosis is made. the chiropractor will advise on which treatment would be ideal for your individual problem.

Hip pain

Lots of causes... The patient’s age is often an important factor in the diagnosis... eg. malformations from birth, slipped epiphysis, Perthe’s disease, osteoarthrosis, bursitis, referred pain from the spine or elsewhere... the list is long. Consulting a chiropractor for an accurate diagnosis helps prevent prolonged, inappropriate treatment. Accurate diagnosis ensures correct treatment, giving fast effective relief of symptoms.

For example bursitis responds best to an injection but severe osteoarthrosis requires an orthopaedic surgeon’s opinion on a possible operation. Many other common hip pains can respond very well to chiropractic treatment.

Perineal pain

Pains in the region of the anus, vagina or testicles

In a neurological sense, this is a very complicated area of the body. Over the past 30+ years Scott has seen many patients with pains, irritation, numbness, and altered sensations in this region. The pains can be in any of the following regions: vagina; labia; anus; rectum; testicle; scrotum; penis; coccyx; etc.

Knee pain

As with hip pain the list of possible diagnoses is long. Many knee pains can respond well to non-invasive treatment but obviously severe osteoarthrosis, arthritis, infections, fractures, ruptured ligaments or severely damaged cartilages will not respond to any conservative treatment so these cases would not be accepted and an opinion from an appropriate orthopaedic surgeon would be the correct way forward.

Plantar fasciitis

We never used to treat this until one of the premiership footballers mentioned he had it when he was seeing Gowan Robinson about his bad back (Gowan has been practising with Scott at the Wilmslow Clinic since 1982).

The player responded easily in two treatments and his club (they play in blue...) telephoned and quizzed us about it! When he explained that he had little interest in the condition but simply treated the player because he mentioned it at the time... they really got quite excited! Apparently it can be very difficult to cure...

Clinical trials have recently proven that chiropractic treatment can be effective for this condition.

Weakness of the Knee or ankle

Knee or ankle giving way

Common complaints - yet they can respond very easily. This is a very good example of why accurate diagnosis and correct advice are imperative before treatment is given.

In many of these cases there is absolutely no point in doing exercises “to strengthen the muscles”... because after accurate diagnosis and treatment, the knee or ankle strength recovers extremely quickly. Only then will the correct exercises will maintain the strength.

This is yet another excellent example of why people should not have more than four treatments from anyone - unless they are getting good results. If in doubt, stop wasting your time (and money) and get a second opinion!

"Wear and tear" & "old age"

We have our limitations! However many patients find that with our gentle precise treatment they “rejuvenate” in the sense that they are less stiff; can put on their shoes and socks more easily; can rotate their necks to reverse the car better; and have less pain.

Please note that they still have the same amount of wear and tear “osteo-arthrosis” and will be exactly the same age... but they may well feel younger!

Arthrosis (Rheumatism)

Not necessarily a painful condition! Simply “wear and tear” or “old age” or a bit of “rust”! If in the hip, knee or ankle - the weight bearing joints - then the joint can be very painful and restricted when moving. In the spine it can be a largely irrelevant X-ray finding and therefore not connected to the pain. The chiropractor will try to find the cause of the pain and then advise which type of treatment would be the most effective.

Arthritis (Rheumatism)

Arthritis is a disease. It is NOT wear and tear - ie. not “arthrosis”. With over 170 types of “arthritis” to choose from, you must be certain exactly which one you have? To accurately diagnose the more obscure types and sub-types of arthritis it is always best to see a consultant rheumatologist.

“Rheumatism” simply means an achy joint. It is not a proper diagnosis. Arthritis is unfortunately not cured by any treatment - including chiropractic. However many people have pains that are not directly associated with the arthritis. They simply have a concurrent mechanical bad back, knee or hip pain that could respond well to chiropractic treatment.

Fibromyalgia

Muscular Aches and Pains

This title covers a multitude of symptoms and conditions. We treat many types of myofascial pain syndromes every day but please note that we are not able to help ME or fibromyalgia.

NB. We do of course happily treat people who have ME or fibromyalgia... but we only treat them for the pains that are coming from other causes.

Osteoporosis & Osteopaenia

Osteoporosis usually needs prescribed medication from the GP or rheumatologist.

Chiropractic cannot help osteoporosis, but of course patients who have osteoporosis can still have chiropractic treatment for many of the other conditions in this list.

NB. If there are any small fractures in the bone - eg a vertebral body crush fracture - the pain often starts suddenly and is sharp and very severe. It can happen spontaneously or after a fall or other accident. Chiropractic cannot help this pain.

Sports injuries

The chiropractors at our clinics have treated many internationals and professional sports people from premiership football clubs; rugby clubs; athletic, hockey, cricket, lacrosse, squash, badminton, martial arts, swimming, (the list is endless) clubs; and of course the “normal” weekend sports people who suffer just as much!

We have attended international and World sporting occasions as “the chiropractor” for the competitors and as very keen (but now totally past it!) competitors ourselves, we really understand the pains; the pressures on the player; and the need for fast effective relief.

We are always happy to work and liase with surgeons, doctors, physiotherapists and other health professionals who treat sports injuries and find that in many conditions, because of the extreme demands put on the athlete’s body, the combination of treatments is far superior to any one approach

Trigeminal Neuralgia

A very painful condition that gives shooting, “lightning” pains in the face. Unlike facial neuralgia, we find in our clinic that this type of neuralgia can sometimes respond well and therefore does not require surgery.

As yet we are unaware of any scientific RCTs (randomised controlled clinical trials) that prove any non-surgical treatment is effective... so we are hoping someone will arrange a clinical trial to prove how successfully chiropractic can cure this severely painful condition. The main cause is described as a vascular irritation of the trigeminal nerve... but this does not fully explain why the pains are infrequent attacks and not a constant or throbbing pain.

Yes... we will happily volunteer for a trial!

Enuresis (Bedwetting)

Many children are brought to Scott for this distressing problem. It can be a 24/7 bladder problem or a problem exclusively at night time in bed - ie. “Nocturnal enuresis”. Every parent knows how much this condition affects the whole family

There have been, as yet, no RCT’s (Randomised Clinical Trials) that have tested the effectiveness of our approach to this condition. However, in-house, we have had some excellent results treating children and teenagers who have had problems with bladder control.

The condition is undoubtedly multi-factorial... and each youngster has to be assessed individually. Parents can bring the child to Scott for a consultation and discussion on the whether his approach might be of benefit.

We often use a mixture of techniques including advice on medication; night alarms; chiropractic; psychology; and acupuncture.

In addition to consulting in his clinics, Scott has travelled internationally to advise and treat children and teenagers suffering with enuresis. He both designed the protocol and is the treating consultant in a three year case series research trial on enuresis, and he is very much looking forward to taking part in a full clinical trial.

Scott has also treated many cases of childhood and adolescent encopresis (encopresis can be defined as "having bowel movements in inappropriate places ie. other than the toilet, after the age when bowel control would normally be expected") with some excellent results.

Gynaecological pain

Please note that we will only see patients after the GP or gynaecologist has confirmed that there is no obvious internal medical problem. We not treat any gynaecological pathology or diseases... eg Polycystic ovary syndrome (PCO); endometriosis; infections; prolapses; etc.

This ensures that the cause of the pain is more likely to be referred from elsewhere and thus could respond well to our treatment.

Patients who consult us, complain of unexplained pain, numbness or paraesthesia (altered sensation) in the vagina or region around the vagina.

Perineal pain

Pains in the region of the anus, vagina or testicles

In a neurological sense, this is a very complicated area of the body. Over the past 30+ years Scott has seen many patients with pains, irritation, numbness, and altered sensations in this region. The pains can be in any of the following regions: vagina; labia; anus; rectum; testicle; scrotum; penis; coccyx; etc.

Pregnant Ladies

Not really a “condition” that needs treatment (!) but lots of “mums-to-be” suffer from musculo-skeletal aches and pains associated with their pregnancy.

Accurate diagnosis and prompt treatment helps these ladies have a much more comfortable time as they count the hours until “the big day”!

Post-partum pain

We regularly check “young mums” to ensure that there are no musculo-skeletal problems resulting from the birth of their new baby. It is a golden opportunity to correct underlying problems as the spine and related joints are so moveable for some weeks after childbirth.

A check at six weeks post-partum is ideal - if there have been no obvious symptoms. But if in pain in the first six weeks, don’t delay. Telephone us immediately to discuss the problem or make an appointment.
